Forms & Availability
The following forms and instructions will be available
outside the ramped side door of the Jones Library during our holds pick-up hours (Monday to Friday, 11 am to 4 pm; Saturday, 2:30 to 4:30 pm), weather permitting. They will be available until they are gone or until April 22, 2021.
- 1040 forms & instructions (redesigned format - to be used for all returns)
- Massachusetts Resident Income Tax booklet - limited supply: 1 per household only
- Massachusetts Nonresident or Part-Year Resident Income Tax booklet
You can also print your own copies of the forms and publications you need – or learn about filing electronically – by visiting the
IRS website and the
Massachusetts Department of Revenue website. To use the Jones Library Print from Home service to print the tax forms you need,

Only the forms and instructions for redesigned 1040 are being supplied to libraries by the IRS this year. Other federal tax forms and instructions can be obtained as follows:
